Who can submit the referral form?

We can only accept referrals from a family member or guardian. If you work with a child, perhaps as a teacher or tutor, and feel that he or she is dyslexic, you should encourage the child's parents to make a referral.

Completing the referral form

Please fill in all details. Your child's school teacher would be required to complete the section for teachers to provide information on your child’s behaviour and performance in school.

Submitting the referral form

Please include the following when sending the completed forms to us:

1. A non-refundable deposit of
   • S$53.50 (i.e. S$50 + 7% GST) for pre-school assessment, as a partial payment for the assessment.
   or
   • S$107 (i.e. S$100 + 7% GST) for full assessment, as a partial payment for the assessment
2. A copy of your child's birth certificate.
3. Any reports that your child may have from psychologists, specialist teachers, speech & language therapists etc.
4. A sample of your child's written work.

Mail the completed Referral Form together with the above to:
